We can also work with your child’s school district and other community agencies to ensure that your family is getting the help needed in every aspect of daily life.

If your child is coming to Catalpa Health for an attention deficit evaluation, these videos will explain more about what to expect during the first visit. Please note that if your child sees a psychiatrist or psychologist right away, the experience will be different.

Forms Needed to Schedule a Psychological or Neuropsychological Testing Appointment

If you need to schedule an appointment for psychological or neuropsychological testing for your child, the process is different. To provide the most appropriate service, we need your completed forms first.

Please print and complete the Patient and Family History form below. In addition, please call our Catalpa Health Access Line at 920-750-7000 to ask what additional paperwork is necessary based on your child’s needs.
